Health and Welfare Benefits
Eligible employees must be regularly scheduled to work a minimum of 20 hours per week.
Medical and Prescription Drug Coverage
Two medical plans, PPO (with FSA), and HDHP (with HSA) eligible for both you and your spouse, and children up to the age of 26.
Dental Benefits
Benefits offered for preventative care and diagnostic services for both basic and major services. Child (up to the age of 26) and adult orthodontia (including Invisalign) coverage available!
Vision Benefits
As a participant, you and your dependent (if you choose family coverage) are eligible to receive the following vision benefits from any vision provider: Up to $250 per calendar year for eye exams and/or glasses or contact lenses.
Flexible Spending Accounts
If you choose the Traditional PPO plan, you can contribute up to $3,300 annually to a pre-tax FSA to cover medical, dental, or vision expenses not covered by other plans. The full amount is available on January 1 and must be used by December 31. FSAs are ‘use-it-or-lose-it’ accounts, but you can carry over up to $660 to 2026.
Health Savings Accounts
If you choose the High Deductible Health Plan, you can contribute up to $3,320 (individual) or $6,950 (family), with SBL adding funds to help reach IRS limits.
Provided Benefits
Basic Life
SBL Provides full-time employees receive life insurance at no cost. The coverage is two times your annual salary, up to $250,000, and will be paid to your beneficiary.
Accidental Death & Dismemberment Insurance (AD&D)
Full-time employees are provided accidental death and dismemberment coverage equal to two times their base salary, with a maximum benefit of $250,000.
Long-Term Disability
Sarah Bush Lincoln offers full-time employees basic Long-Term Disability (LTD) coverage at no cost.
Voluntary Benefits
Accident
Accident Insurance from Reliance Standard helps cover out-of-pocket costs for accidents, both on and off the job. It provides benefits for ambulance rides, ER visits, doctor appointments, fractures, burns, surgery, medical equipment, and more.
Voluntary Term Life Insurance
Employees can choose up to $250,000 in Voluntary Term Life Insurance. Rates depend on age and are shown on Workday. New hires can get up to $250,000 without medical proof if they enroll within 30 days.
Accidental Death & Dismemberment Insurance (AD&D)
The Plan also allows you to purchase Voluntary Accidental Death and Dismemberment (AD&D) Insurance for yourself, your spouse, civil union partner and your unmarried dependent children birth to 26 years.
Short-Term Disability
As a full-time or part-time employee, you can purchase Short-Term Disability (STD) coverage to protect your income. The STD plan provides weekly payments of up to 60% of your base earnings. You can choose between two coverage options for added flexibility.
• Coverage beginning after 14 days of disability
• Coverage beginning after 30 days of disability
Critical Illness
Critical Illness Insurance through Reliance Standard helps protect your family and assets during tough times. If you or a covered loved one is diagnosed with conditions like cancer, heart attack, or stroke, the policy provides a lump-sum cash benefit.
Hospital Indemnity
Hospital Indemnity Insurance through Reliance Standard helps protect your finances by paying a daily benefit for each day you’re confined to the hospital, along with a lump-sum admission benefit. The plan pays benefits directly to you, in addition to your medical plan, giving you flexibility to use the funds as needed. You can cover out-of-pocket hospital expenses or other costs, like groceries or childcare.
Work/Life Balance
Time Off Paid (TOP)
The TOP program provides paid time off for vacations, illness, and emergencies. Full-time and part-time employees earn TOP time based on their length of service. Starting will receive 23 days a year.
TOP hours begin accruing immediately upon employment for eligible employees. You can use TOP hours for holidays, up to the amount you’ve accrued.
Reserve Sick
The Reserve Sick time account provides protection from pay losses due to an employee’s own hospitalization, long-term serious medical problems, outpatient surgery, oral surgery, invasive procedures, procedures requiring sedation and open or closed reduction of fractures. A physician note is required to access Reserve Sick.
Tuition Assistance
As a full-time or part-time employee, after completing your trial period, you can receive tuition reimbursement for approved courses. Courses must be from an accredited institution and related to your job or career path.
Adoption Assistance
Adoption assistance is available to employees who have been with the company for 12 months in a benefit-eligible status. Full-time employees can receive up to $5,000 in reimbursement, and part-time employees up to $3,000.
Student Debt Program
SBL will provide eligible employees with a company funded payment to employee qualified higher education student loans to help reduce student loan debt.
Wellness Benefits
Wellness & Health Plan Rewards
Employee who participate in our Wellness Program, and are enrolled in one of our health plans, can receive funds loaded onto an HRA/ PBA Account. SBL will contribute to the employee’s account based on employee and spouse (if applicable) participation.
Funds Loaded:
• $200 Employee
• $100 Additional for Spouse (if applicable)
• (Employees enrolled in EE & Child coverage level will automatically earn the additional $300 for employee participation.)
Funds contributed to the Personal Benefit Account will be loaded to the Benny Card and may be used to pay for out-of-pocket expenses related to health, dental or vision care.
Employee Assistance Program (EAP)
The Sarah Bush Lincoln EAP staff, consisting of behavioral health professionals, can assist with various issues, including: Sarah Bush Lincoln (SBL) covers the cost of EAP services as a benefit for employees and their families, providing each person with up to 10 hours of service per year. EAP services extend to your family members living in the same household as you.
ComPsych
ComPsych Guidance Resources benefit offers 24/7 support for you and your dependents, providing confidential counseling, legal assistance, financial resources, and personalized work-life help for all of life’s changes.
Additional Benefits
PerkSpot
SBL is excited to offer PerkSpot, a convenient platform for exclusive discounts at many national and local merchants. PerkSpot is free to use and works on desktops, tablets, and phones.
Employee Referral Program
SBL offers a referral program where you can earn a $100 reward for referring a friend who is hired. It’s a great opportunity to share the benefits of working at SBL with others and receive a bonus when your referral is successful!
Gainsharing Program
Employees are rewarded with an incentive bonus when Sarah Bush Lincoln reaches established goals. Goals center around overall financial performance, patient experience and employee retention – in achievement of our strategic priorities. The annual maximum payout is $1,500. Eligibility criteria include date hired, hours worked and active employment.
Retirement Benefit
SBL provides a qualified defined contribution retirement plan for you. All employees may begin making contributions to the retirement plan from their date of hire. Participating employees become eligible for the employer match the first of the month following 30 days of employment and attaining age 18.
The match is equal to 50 percent of your contribution, not to exceed 4 percent.
